• Patient Survey: Bronchiectasis and COPD

  • Thank you for your interest in participating in this survey. We are collaborating with Insmed Incorporated (a pharmaceutical company) on a project to better understand the patient journey for individuals diagnosed with COPD and/or bronchiectasis.

    You are not required to provide personal information in your responses to the questions in this survey. Your responses are not linked to your COPD360social, BronchandNTM360social, Facebook, or Instagram account.

    By completing the survey, you acknowledge that the COPD Foundation/Bronchiectasis and NTM Association has your consent to analyze the responses (data) and to potentially publish the de-identified results in a medical journal. No personal information about you will be shared publicly or included in potential publication of the results of this research, and your responses will not be able to be linked back to you. Your participation in the survey is completely voluntary, and you are not required to share any information that could identify you.
